Now Hiring: Licensed Vocational Nurse (LVN)
Position Summary
The Licensed Vocational Nurse (LVN) delivers direct nursing care under the supervision of a Registered Nurse or Physician, following the California Nurse Practice Act, Title 22 regulations, and facility protocols. The LVN provides high-quality, compassionate care to residents with complex medical needs, including those requiring ventilator support, TPN administration, wound care, and hospice-level services.
Key Responsibilities
Clinical Duties
- Administer medications and treatments within the LVN scope of practice, as prescribed.
- Monitor and report changes in residents’ conditions to the RN or physician promptly.
- Perform wound care, tracheostomy care, and tube feeding as trained and delegated.
- Collect specimens for diagnostic testing as needed.
- Respond to medical emergencies and assist in resuscitative efforts.
- Adhere to infection control policies in line with facility protocols and Title 22 standards.
- Provide input for resident care plans through ongoing observation and participation in assessments.
- Implement care plans as directed, including assistance with activities of daily living (ADLs) and other nursing interventions.
- Deliver care that is both patient-centered and culturally sensitive.
Qualifications
Required:
- Active California LVN license in good standing (BVNPT)
- Current Basic Life Support (BLS) certification
- Minimum of one year of clinical experience, ideally in subacute, long-term care, or home health
- Familiarity with electronic medical records (EMR/EHR) systems
- TB clearance and physical exam as required by Title 22
Preferred:
- Experience in CLHF, SNF, hospice, or ventilator care
- IV certification (per facility policy)
- Bilingual (English/Spanish or other languages) is a plus
Physical & Work Requirements
- Ability to lift/move up to 50 lbs
- Frequent walking, standing, bending, and patient mobility assistance
- Exposure to infectious materials and typical clinical hazards
- Availability for rotating shifts, including weekends and holidays

This role involves caring for clients in alignment with individualized care plans under the supervision of a Registered Nurse (RN) or licensed healthcare professional, and in compliance with all relevant federal, state, and local regulations.
Key Responsibilities and Duties
- Assist patients with activities of daily living, including bathing, dressing, and grooming.
- Monitor and record vital signs such as temperature, pulse, and blood pressure.
- Accurately document patient information using electronic health record systems (e.g., Epic).
- Provide basic life support and assist in emergency situations as needed.
- Support nursing staff with implementing care plans and administering treatments.
- Safely lift, reposition, and transfer patients to ensure comfort and prevent injury.
- Participate in care activities that promote patient well-being and recovery.
Required Skills
- Strong understanding of caregiving principles and practices.
- Basic Life Support (BLS) certification required; additional certifications are a plus.
- Ability to lift and move patients safely while adhering to safety protocols.
- Excellent communication skills to engage effectively with patients, families, and the healthcare team.
- Knowledge of infection control practices to maintain a safe care environment.
